Outpatient Information
On arrival, please check in at our Reception. It is important that you bring the following to your appointment:
Your appointment letter (if applicable)
A valid form of identification (ID card, Driver’s Permit, Passport)
Medical reports or documents – Any documents pertaining to your medical problem
Insurance Form (if any)
Insurance card (if any)
Funds to cover your admission charges (cash, Linx, credit card, certified cheque)
Your regular prescription list (if you have one)
Any tablets or medicines that you are currently taking, including those which may be for any non-neurological conditions you have, for example asthma, heart disease. Particularly if you have to take your medicine at certain times of the day.
Valsayn Medical aims for patients to be seen on time. If your clinic appointment is delayed, you will be notified as soon as possible.
